Appendix D: Website Supplement For Users of the 3sHealth Website

1. Collection of Personal Information

3sHealth may collect personal information (PI) from website users where the user chooses to provide that information via 3sHealth's website. 3sHealth has implemented safeguards to protect the information collected via its website. However, the transmission of information via the internet is inherently insecure. As such, please do not submit sensitive information via the 3sHealth website.

Visitors to 3sHealth’s website should also be aware that anonymous technical information may be collected by 3sHealth as a result of a visit to 3sHealth’s website.
