Year-end data is currently being confirmed, and T4 forms will be posted to MyConnection on February 13. To access tax documents, health workers must log in to MyConnection, click on the MyWork drop-down menu in the upper-right hand corner of the site, and select MyWork. Click on the “Pay” tile from the Apps section, then the “Year-End Documents” tile.

Former employees of health-system partner organizations will receive a copy of their T4 by mail from their employer.

T4As for benefit plan members with 2025 disability income earnings

Benefit plan members who require a T4A, the form that summarizes a member’s short-term and/or long-term disability income, will receive those documents in the mail. They will be sent out by the end of February. Those looking for access sooner can also find their T4A in their CRA account as soon as the agency uploads them.
