Applying for disability benefits
To apply for disability benefits, there are three forms that must be completed and sent to 3sHealth:
- Employer’s Initial Application Form (to be completed by the employer);
- Employee’s Initial Application Form (to be completed by the employee applying for benefits); and
- Attending Physician’s Initial Statement Form (to be completed by the employee’s physician).
All forms must be signed and dated in order for the application to be processed.
