Dealing with the loss of a loved one can be very overwhelming. This section contains information for plan members, next-of-kin, and executors. Find information on life insurance, keeping beneficiaries up to date, and more.

We hope to make the process of reporting a life insurance claim easier for everyone involved by answering some frequently asked questions below that may be helpful at this difficult time.

Life insurance benefits

Basic Life insurance equals two times your annual salary, up to a maximum of $1,000,000. Dependent Life insurance coverage is $10,000 on the life of your spouse and $5,000 on the life of each of your eligible dependent children payable to the plan member. The Group Life Insurance Plan automatically includes Dependent Life insurance as part of Basic Life insurance at no additional cost. 

For injuries sustained by an accident, your Basic Accidental Death & Dismemberment (AD&D) insurance equals a percentage of your Basic Life insurance. The plan bases coverage on the current “table of losses.” The table of losses details how much coverage plan members have in the event of a loss of life, limb, sight, hearing, or speech. See the Group Life insurance booklet for the table detailing the loss coverage on page 10. The premiums you pay for Basic Life insurance cover the cost of AD&D insurance.

Your Group Life insurance is only available upon your passing, unless you have a terminal medical diagnosis. In the event your physician diagnoses you with a terminal illness, you may request an “Early Death Benefit” payment of up to 50 per cent of your basic Group Life insurance coverage or $50,000, whichever is less. To be eligible, you must have a terminal diagnosis with a life expectancy of less than 24 months. Please contact 3sHealth to advise that you will be applying. We will provide forms for you and your physician to complete. An approved Early Death Benefit application reduces the Basic Life insurance amount payable to your beneficiaries by the advanced amount. If you survive past the 24-month life expectancy, you do not need to pay back the advance; the advanced payment amount will reduce the Basic Life insurance amount payable to beneficiaries upon your death. 

While alive, a plan member may have purchased additional Optional Life insurance. Here is an overview of what may be available. Check your records or contact 3sHealth to see what coverage a deceased plan member may have.

Optional Life insurance beyond Basic Life insurance is available. A plan member can purchase Optional Life insurance in units of $10,000 up to a maximum of $500,000. This additional coverage requires medical evidence of insurability. Within the first 90 days of plan membership, you can elect to purchase up to $150,000 of Optional Life insurance without medical evidence. Optional Life insurance premiums are based on your age and on smoker status. The monthly rates for Optional Life insurance are available here.

Voluntary Accidental Death & Dismemberment (AD&D) beyond Basic AD&D is available. As a plan member you can purchase Voluntary AD&D in units of $10,000 up to a maximum of $250,000. The plan bases coverage on the current “table of losses.” The table of losses details how much coverage plan members have in the event of a loss of life, limb, sight, hearing, or speech. Making a life insurance claim

We are sorry for your loss. To start the process of making a life insurance claim, please contact 3sHealth.

Once we have confirmed the eligible life insurance coverage, we will reach out to the named beneficiaries directly. We will then send a package outlining which documents the beneficiaries must submit, as well as application forms each beneficiary needs to complete and sign. When the beneficiaries have returned the documents to us, we will process the claim and then Canada Life will pay the proceeds directly to each beneficiary.

If the plan member had active health and dental coverage it will continue for the surviving spouse and eligible dependent children for up to two years from the plan member’s date of death or until the surviving spouse remarries, whichever is earlier.

We are sorry for your loss. If you are enrolled in the Group Life Insurance Plan, you have coverage for an eligible spouse in the amount of $10,000.

To start the process of making a life insurance claim, please contact 3sHealth. We will send you forms to complete. We will be here supporting you through the full process of making the claim.

You may also want to make changes to your beneficiaries for your Group Life insurance coverage. You can simply do this by completing the “Group Life Beneficiary Designation Form.” Please send the completed form to ebp@3sHealth.ca.

We ask that you also complete the “Employee Benefits Dependent Change Form” to remove your spouse from your health and dental plan. Send the completed form to ebp@3sHealth.ca

We are sorry for your loss. If you are enrolled in the Group Life insurance plan, the plan includes coverage for eligible children in the amount of $5,000.

We are sorry for your loss. 

If the plan member retired prior to Jan 1, 2015, they may have received a $1,500 “Paid-up Life Insurance Policy” certificate, payable to their beneficiaries upon the plan member’s passing. Please contact 3sHealth to confirm if we issued a certificate and to start the claim process, if applicable.

If the plan member retired after Jan 1, 2015, they may have received a $5,000 “Retired Plan Member Life Insurance Benefit (RIB)” certificate, payable to their beneficiaries upon the plan member’s passing. Please contact us to confirm if we issued a certificate and to start the claim process, if applicable. 

Once we have confirmed that the plan member was eligible, we will reach out to the named beneficiaries directly. We will then send a package outlining the documents and a claim form that each beneficiary must complete. Once the beneficiaries have returned the documents to us, we will process the claim and pay the proceed to the beneficiaries.

If the beneficiary who passed away was your spouse, you may be eligible for a Dependent Life insurance claim if you elected to continue your basic Group Life insurance. Please contact us to confirm if your claim is eligible.

Helpful tip: You should also contact your pension provider to inform them of your spouse's passing.

If you have a “Retired Plan Member Life Insurance Benefit (RIB),” we will send you a letter confirming the change to your beneficiary. Please keep this letter with your original RIB certificate, as we cannot issue a new certificate.

You can name any person as your beneficiary, such as your spouse, parent, child, or anyone else in your life. You can also designate more than one beneficiary. You can also name a legal entity as your beneficiary, such as a charitable organization. If you choose to name a charitable organization as a beneficiary, 3sHealth requires the full legal name, address, and the charity registration number of the charitable organization.

Death of a plan member on disability income benefits

We are sorry for your loss. The plan’s disability benefits end on the day the plan member passes away. For this reason, it is important to contact 3sHealth right away to ensure no overpayment occurs. 

There may also be Group Life insurance coverage in place. Once we have confirmed the eligible life insurance coverage, we will reach out to the named beneficiaries directly. We will then send a package outlining which documents the beneficiaries must submit. When the beneficiaries have returned the documents to us, we will process the claim and then Canada Life will pay the proceeds directly to each beneficiary.

If the plan member had active health and dental coverage it will continue for the surviving spouse and eligible dependent children for up to two years from the plan member’s date of death or until the surviving spouse remarries, whichever is earlier.

Resources for estate executors

Contact 3sHealth to confirm eligible life insurance policies for the deceased plan member.
